Sir I have the honor to apply for permission to appear before one of the Medical Examining Boards established according to Genl Order No "34" App[torn away]
Feby 17th 1864 to be retired from Field Service to duty because of deformity of leg & arm caused from a protracted case of fever which renders me entirely unable to perform the duties of a soldier in the field
