I enlisted at Urbana Ill. June 1st 1861 under Capt. E. Boyden, was mustered into the Service of the U.S. Aug. 9th 1861 by Capt Albert Tracy U.S.A. at St Louis Mo. for 3 years. As 4th Sergeant in Co. "K" was with the Regt. on all the long & fatigueing marches through Mo. under Gen Fremont, was engaged in the long & hotly contested battle of Pea Ridge Ark. under Gen Sigel; soon after was assigned to Gen. J. C. Davis Division where I have since remained. Was in the Rifle pits before Corinth when that place was evacuated by Gen Beauregard; was on the long & toilsome march from Jacinto Miss. & Louisville Ky. was in reserve at the battle of Perryville Ky. Oct. 8th '62 marched by way of Crab Orchard to Nashville had a skirmish with Morgans Gurrilleas Nov. 8th '62. At Tyree Springs Tenn; also at Nolinsville Dec. 26th '62 & was wounded in front of Murfreesboro Dec. 30th '62 by a musket ball which entered my body 3 inches to the left of the spine & came out about 6 inches to the right of the spine / rendering me unfit for further field service, & I respectfully ask a transfer into the Invalid Corps which is now being organized
I was promoted from 4th to 2nd Sergeant from that to Capt. soon after the battle of Stone River for Meritorious Service. have never been absent from Regt. till after I was wounded. Since that time I have been absent from the Regt. near half the time & have done but little duty when present, as particles of bone continue to work out, the 10th rib being broken, the last piece of which came out on 14th of this month.
My residence is Urbana Ill. am married now 27 years of age, 5 ft 8½ inches high, was born in Crawfordsville Indiana.

I hereby certify that Capt. Jas. M. Tracy was wounded on the 30th day of Dec. 1862. by a musket ball & that the wound & present condition are such as described in this paper & that this wound was received while in the line of duty. also that the wound disqualifies him from field service but not from the service of the Invalid Corps.
