Adams of the 2^nd Regt TMR at Fort Inge on the 8^th day of June 1861 to serve one year. he was born in Freiburg County Switzerland, is 20 years of age, 5 feet 6 inches high light complexion, blue eyes light brown hair and by occupation when enlisted a farmer. During the last 2 months said soldier has been unfit for duty

 

I certify that I have carefully examined the said Jacob Ettai of Capt W. C. Adams's company and find him incapapble of performing the duties of a soldier because of being blind of the left eye, and an affection of the left leg caused by a fracture of the bone when quite young he is also subject to epistaxis daily which keeps him constantly weak and unfits him to perform military duty
