            Enclosed herewith please find a communication from Mrs. Jane S. Hotchkiss received at this office on the 13th inst. Mrs. Hotchkiss is an applicant for pension on account of services and death of her husband Norman Hotchkiss late of Co. "B" 13th Conn. Vols. and the Department requires proof as to the date and cause of the soldiers death from some person having personal knowledge of the facts. The proof should be furnished in the form of an affidavit and must be sworn to before a Court of Record or if before a Notary or Justice it must be accompanied by a certificate of authenticity. If you can furnish such proof or the address of some person who was with him at time of decease you will confer a favor by forwarding it to this office as soon as practicable.
