upon Honor that on the sixth day of April A.D. 1862 I was acting as 2d Lieutenant of Company E in the 54th Regiment O.V.I. That on said day while Lewis Burnham was a 4th Sergeant in said Company in the service of the United States in the line of his duty at the battle of Pittsburgh Landing he was wounded in the left thigh by a ball passing through the thigh rendering him unfit to perform military Service
