To all whom it may Concern:
Know ye, That George W. Yates a Lieut. and of Captain Adjutant Company 4th Regiment of Michigan Infantry VOLUNTEERS who was enrolled on the 20th day of June one thousand eight hundred and 61 to serve three years…is hereby DISCHARGED…this____day of June 1864 at Detroit Michigan by reason of Expiration of time of Service
 
            Said George W. Yates was born in Albany in the state of New York, is 22 years of age, 5 feet 11 inches high, Light complexion, Blue eyes, Auburn hair, and by occupation, when enrolled, a Clerk
Given at Detroit Mich this 28th day of June 1864.
