Dear Madam:
I take pleasure in informing you that your claim for pay and allowances due you according to the amendment of your late husband's muster into service as an officer of Vols. has been reached for settlement. The Auditor has requested me to furnish him with a certified transcript from the Church or Public record of your marriage to the officer, and also a similar transcript showing the date and place of his death; or if you are a pensioner and will send me the number of your pension certificate I will use the proof of marriage and death on file in that case. This will not interfere with your pension. Please send me the affidavit of one of your late husband's brother officers, or two enlisted men aware of the facts, showing that the disability on account of which the officer's resignation was tendered and accepted, was received in service and in line of duty. If you can furnish an affidavit showing the place at which the officer entered the service, where he was discharged and whether he paid for his transportation and subsistence from the place of discharge to his home, please advise me to that effect. Enclosed blank receipts are to be signed by you in the presence of two attesting witnesses and returned to me at once for filing. It is necessary for you to do this in order that you may get the money that may be due you in this case.
Please attend to this matter at once.
