I certify on honor that during the month of May 1864 the stores enumerated below were lost under the following circumstances—
The regiment to which my company belongs were engaged with the enemy on the 10th 12th & 23d days of May 1864. during the engagements I lost 4 privates killed, 2 noncommissioned officers and 5 privates wounded. the arms and equipments carried by these men: for the want of transportation were abandoned. The following is a list of stores so abandoned—
