To
Brig. Gen'l A. Asboth
                                                                        Commanding District West Florida.
The petition of the undersigned respectfully showeth: That your petitioner, being yet at large, and free from any confinement or duress, voluntarily comes forward and desires to take the Amnesty Oath, with the purpose of restoring Peace, and establishing the National Authority, and to claim the privileges thereto belonging, according to the true intent and meaning of the Proclamation of the President of the United States in relation thereto; that your petitioner makes this application in good faith, with the true intent and purpose on his part, without any reservation whatever, of faithfully and loyally supporting the Government of the United States; and your petitioner further states that the following is a true, complete and distinct statement of all the services, and aid of any kind, that he has rendered to the rebel authority, and from the penalties of which treasonable acts, he asks to be relieved, viz:

I am 24 years of age my home is in Coffee Co. Ala. I was conscripted in August 1862 and was kept in "Camp Watts" in Macon Co. Ala. for about six weeks at which time I was discharged and sent home I have taken no further part in the late Rebellion against the Authority of the United States /

And your petitioner is fully and distinctly aware, that any material false statement in this, his petition, will invalidate the protection of the Amnesty, at any time when discovered, and subject the party, making such false statement, to trial, for his former treasonable acts.

And your petitioner doth solemnly swear that each and all of the foregoing statements, on his part, are true and in good faith, and he takes this oath voluntarily and without any mental reservation whatever—So help him God.
